Job Title & Role Description:

The Manager, Clinical Operations leads multidisciplinary Care Teams providing in-home care to high-risk patients. Reporting to the Vice President of Clinical Operations, this role focuses on managing care delivery in value-based environments, reducing hospitalizations and ED visits, and increasing access to care. Key responsibilities include hiring and onboarding staff, ensuring adherence to clinical protocols, and fostering team development.

This role drives performance by managing caseloads, monitoring key performance indicators (KPIs), and leading quality improvement initiatives. The Manager ensures compliance with healthcare regulations and contractual agreements while continuously improving processes for patient onboarding, care delivery, and reassessments. Collaboration with internal and external partners is critical to achieving positive patient outcomes and operational efficiency.

Skills Required:

  • RN required; additional experience in healthcare administration or policy preferred.
  • Minimum of 2 years of leadership experience in a clinical operations environment, with a focus on value-based care.
  • Proven experience in case management and driving patient engagement to improve health outcomes.
  • Experience managing multidisciplinary care teams and implementing patient-centered care.
  • Proven track record in analyzing data, making decisions, and implementing improvements in a fast-paced setting.
  • Knowledge of quality metrics for value-based care and experience working with Medicare and Medicaid populations.
  • Excellent interpersonal skills with the ability to influence and interact with clients and community partners at all levels.
  • High level of flexibility and adaptability, particularly in an evolving, rapidly growing company.
  • Strong communication skills, both written and oral, with the ability to communicate proactively and clearly with team members. Experience in managing Medicare and Medicaid populations preferred.
  • Ability to travel throughout the market, with occasional travel within the US.
